Adenosine to facilitate the clipping of cerebral aneurysms: literature review
نویسندگان
چکیده
Cerebral aneurysms have a high mortality rate when ruptured. Endovascular techniques have improved substantially in treating this pathology. However, surgical clip ligation remains the preferred option for some aneurysms. Various techniques are used intraoperatively to assist the surgeon in dissecting the aneurysmal dome free of surrounding tissue and placing a clip around the neck safely and effectively so that no nearby perforating vessels are affected and no residual remains. These techniques include temporary clip ligation, endovascular balloon occlusion and cardiac standstill. Adenosine use is one viable option for induced cardiac arrest leading to a short period of controlled hypotension. Its predictable course of action, rapid onset and offset and rare incidence of adverse side effects make it an attractive agent in this regard. Below, we provide an introduction to adenosine use, describing its pharmacokinetic properties, indications, contraindications, complications and future directions.

Adenosine-induced transient asystole for intracranial aneurysm surgery: a retrospective review.
UNLABELLED BRIEF SUMMARY: We describe the use of adenosine-induced cardiac arrest to facilitate intracranial aneurysm clip ligation. BACKGROUND Cerebral aneurysms are highly variable which may result in difficult surgical exposure for clip ligation in select cases. Secure clip placement is often not feasible without temporarily decompressing the aneurysm. This can be accomplished with tempora...
متن کاملSeven Intracranial Aneurysms in One Patient: Treatment and Review of Literature
Before the advent of endovascular coiling, patients with multiple intracranial aneurysms were treated with surgical clipping; however, with the advancements in endovascular technology, intracranial aneurysms can be treated with surgical clipping and/or endovascular coiling. We describe a case of subarachnoid hemorrhage in a patient with 7 intracranial aneurysms. A 45-year-old female developed a...
متن کاملDelayed symptomatic vasospasm after clipping of an unruptured intracranial aneurysm: case report and literature review
Symptomatic vasospasm after clipping of unruptured intracranial aneurysms is a rare phenomenon. Herein, a case of delayed symptomatic vasospasm that developed after clipping of a middle cerebral artery aneurysm is reported. The experience in treatment is introduced, and relevant literature is reviewed to investigate the potential pathogenesis. A 50-year-old male was diagnosed with an unruptured...
متن کاملEndovascular and Surgical Options for Ruptured Middle Cerebral Artery Aneurysms: Review of the Literature
Middle cerebral artery (MCA) aneurysms are common entities, and those of the bifurcation are the most frequently encountered sublocation of MCA aneurysm. MCA bifurcation (MBIF) aneurysms commonly present with subarachnoid hemorrhage (SAH), are devastating, and are often lethal. At the present time, the treatment of ruptured MBIF aneurysms entails either endovascular or open microneurosurgical m...
متن کاملManagement of Aneurysms of the Proximal (A1) Segment of the Anterior Cerebral Artery
OBJECTIVE Aneurysms originating from the proximal segment (A1) of the anterior cerebral artery are rare; however, because of their small size, the risk of injury of perforating arteries, and the location of the aneurysm in the surgical field, they are challenging to treat. We report on 15 patients with A1 aneurysms and review surgical views according to the direction of aneurysms. METHODS Fif...
